During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We kindly request you access the Patient Portal in advance of your appointment to fill out as much information as possible. This streamlines the process to give you the most efficient visit possible. If you are unable to use the portal, we kindly request you print and fill out the new patient paperwork below prior to your visit.
Compassionate, Comprehensive Care of the Foot & Ankle. Our practice provides comprehensive foot and ankle care, we specialize in foot and ankle surgery, sports medicine, arthroscopy surgery, and general podiatry. Our primary goal is pain relief and restoration of function.
Your first visit to our practice establishes a meaningful foundation. We ask that you arrive at the time requested. You will need to bring the following items:
- Photo Identification (i.e. State issued I.D., Driver's License, Passport)
- Insurance Card
- X-rays, MRI's and any other pertinent medical information
- Complete medication list
- Form of Payment (cash, check, credit or debit card)
Patient Portal for existing patients (If you are a new patient, you will need to be scheduled an appointment in order to set up the patient portal)
